It's ironic, but crashes are making flying safer, and this book again demonstrates Henry Petroski's (my favorite civil engineer) aphorism that we learn more from failure than we do from success. Adair has painstakingly reconstructed the investigation into the crash of a Boeing 737, one of the world's safest airplanes. It was Boeing engineers who discovered the peculiar flaw in the special and unique design of the power control unit (PCU) that caused the rudder to go into hard over - a full appli [...]

In 1994, USAir Flight 427 was in the process of its takeoff from Pennsylvannia, then 40 seconds later it dive-bombed full speed into a hillside - killing all 132 persons onboard. This is the story of a crash investigation that seemed unsolveable. Tensions between the pilots' union and the manufacturer (Boeing) colored the investigation for more than two years. The cause of the accident finally solved, the book shows us the tensions at play in a NTSB crash investigation. Enjoyable read.

This is an interesting book detailing the investigation into the crash of USAIr flight 427. Flight disasters have always interested me and I don't find them particularly unnerving because of the fact that they are so rare. This appears to have been a very difficult case to solve albeit a very important one as the cause of the crash was likely responsible for other crashes which had been unsolved prior. One of the important things to take away is how accidents serve to improve safety of future fl [...]

Always loved flying and aviation and feel it is the most reliable and safest means of transportation we can utilize, however, a very small percentage of times a terrible and catastrophic event happens with flying resulting in death and devastation that becomes a mystery of what went wrong and what can be done to avoid such events happening for future travelers. It is the responsibility of the dedicated and meticulous investigations from the men and women of the National Transportation Safety Boa [...]

I knew some of the history of this aircraft accident so when I saw this book in the library I grabbed it hoping to find out more details about what caused it. Not only did I find out the very detailed cause but of the whole process that went into aircraft accident investigations. What a fantastic read this was! There is such a dynamic process involved with the NTSB, FAA, ALPA, Boeing and the airline that had the accident, in this case USAir. I never realized how they all worked both together and [...]

Similar to others who have submitted reviews, I too am an avid reader of books about aircraft accident investigations. This is one of, if not the best non-fiction book I have read on this subject. It is heartening to know the extents to which the US NTSB will go to definitively determine the probable cause of aviation accidents. This particular investigation led to improvements to the rudder control systems on the Boeing 737, the most popular commercial airliner to date.

I've reread this book a couple of times over the years and it's coverage of the NTSB investigation from start to finish and the accident itself makes me come back to it every few years. There's some parts of the book that are difficult to read but with the subject matter that's to be expected.

Very informative book that shed light on the complexities of investigating an air crash. Learnt a lot about humanity in the real-life accounts. At the end of the day, it all boils down to dollars and cents.

If you are a geek about sysstems analysis, airlines, and yiou like a good mystery similar to "Fate is the Hunter," then this book will please.
